What is gum disease?

Gum disease, also called periodontal disease, is an infection of the gum tissues, and is something seen all too often by Dr. Vincent Cardinale and Dr. Chris Moody. Extending from inflammation of the gums (gingivitis) to more serious infections and complications (periodontitis), there is a wide range of gum disease severity.

Not only does gum disease affect the health of your mouth and teeth, but according to the National Institute of Dental and Craniofacial Research, it can affect your general health as well. This is because an infection in the mouth as a result of gum disease can travel to other parts of your body through the bloodstream. Gum disease is also a risk factor for heart disease, and can play a role in blood sugar levels.

Causes and Risk Factors

Gum disease is essentially caused by the build-up of bacteria in your mouth. If you brush and floss every day, this bacteria is washed away, but if not, it turns into plaque. If left unchecked, this plaque buildup can lead to gum disease.

Some of the common risk factors for gum disease include not taking good care of your teeth, failing to have one’s teeth cleaned every six months, experiencing hormonal changes, smoking cigarettes, developing diabetes, being genetically exposed to gum disease, or taking certain types of medications.

Gingivitis versus Periodontitis

There are two main types of gum disease: gingivitis and periodontitis. Both are bad for you, but gingivitis is less severe. It is typically the first stage, and involves inflammation of the gums from plaque and tartar on the teeth. If your gums are swollen and bleed, this is a sign of gingivitis.

Periodontitis, a more severe case of gum disease, occurs when your gums pull away from the teeth and pockets form. These pockets are a concern because they can harbor infection.

Treatments for Gum Disease

Treatments for gum disease depend on the cause and severity. Deep cleaning to remove the plaque underneath the gum line – called root scaling and planing – is one of the most common treatments for gum disease. Antibiotics placed under the gums to rid you of an infection or reduce the inflammation may also be advised. In some cases, surgical procedures, including flap surgery and bone and tissue grafts, are needed.

The Safety of Dental X-Rays

An article was released to the public stating that dental X-rays contribute to a type of brain cancer. After reading an article like this, your first thought may be to avoid dental X-rays, but you may want to hold off on that quick judgment. As with any treatment we offer at Cardinale Dentistry, education is your most valuable tool in deciding what is best for you.

How often dental X-rays are taken is based on risk for infection, physical symptoms, and clinical findings. The American Dental Association (ADA) is a governing body over the dental profession. The ADA states, “ . . . healthy adults receive routine mouth X-rays every two to three years. Dental X-rays are recommended every one to two years for children and every 1.5 to three years for teens. Children often require more X-rays than adults because of their developing teeth and jaws and increased likelihood for cavities.”

A "caries risk category" often determines how often dental X-rays are taken. The most recent documented resource to determine a caries risk is Caries Management by Risk Assessment (CAMBRA). This was adopted by the ADA and is used by dental professionals giving interval recommendations for X-rays.

With knowledge of your risk for dental infection, you will be informed by Dr. Vincent Cardinale and Dr. Chris Moody of the interval at which dental X-rays should be taken. You can rest assured that the standards published by the ADA have been researched extensively and are there to protect your personal health and safety.

Dental X-rays are most commonly digital, which significantly reduces exposure. There is more radiation exposure from the sun or in an airplane than in a dental X-ray. It is common practice to use a lead apron with a thyroid collar for protection during X-ray exposure.

Heart Disease and Oral Health

Is there a link between oral health and heart health? This is a question that has been receiving a lot of attention in recent studies. While as yet there is no definitive study that proves oral health has an effect on overall heart health, there is also no definitive proof that oral health doesn’t affect heart health. So, what do we know, and, just as important, what should we do?

What do we know? One clear connection exists between gum disease and endocarditis, a rare, but potentially very serious, infection. When we neglect our gums, they bleed more easily. When gum disease advances, the gums pull away from the teeth. This creates pockets which can harbor more bacteria and plaque, leading to infection and even tooth and bone loss. And this bleeding and infection can be serious not only as a matter of dental health, but for your heart as well.

Endocarditis is an infection of the inner lining of the heart and the heart valves. When bacteria from other parts of the body, including your mouth, make their way into the blood stream, they can attach themselves to damaged areas of the heart. The resulting infection can become very serious, very quickly. If you have damaged heart valves or artificial valves, have an artificial heart, or have any type of heart defect, it is especially important to prevent gum disease from developing. Regular dental care will help prevent bleeding gums and infections, and reduce the danger of oral bacteria entering the bloodstream. What else might link oral health and heart disease? Because an increased risk of heart disease has been found in patients with moderate or severe gum disease, there has been much speculation as to a possible cause-and-effect connection. For example, the plaque inside arteries which leads to heart attacks has been found to contain certain strains of periodontal bacteria. Other studies have searched for a causal relationship between inflammation resulting from gum disease and inflammation leading to heart disease.  The body’s immune response to periodontitis and its potential consequences for heart health have also been examined. So far, there has been no clear evidence that treating gum disease prevents the occurrence or recurrence of heart disease.

So what should we do? This is the easy part. Keeping your gums and teeth healthy will increase your quality of life in immeasurable ways. And it’s not difficult! Can You Stop a Cavity Before It Starts?

You’re busy getting ready for your day, and of course you’re starting your day off right by looking after your dental health. A two-minute brushing, some careful flossing, and—wait, is that a white spot on your tooth?

If your mirror reveals a patch of white on your enamel, it could be a symptom of early tooth decay.  Does this mean a cavity is necessarily in your future? Perhaps not, if you act promptly to reverse the damage!

Cavities, after all, don’t suddenly appear in our mouths. Tooth decay is a process, and it begins with a weak area in the enamel that often appears as a white spot on the tooth. These weak spots are known as “incipient cavities,” because they can easily become cavities if left untreated.

Why do these weak spots occur? And how can we repair them? Let’s look at some tooth science.

  • Demineralization

Your tooth enamel is one of the marvels of the human body. Enamel is mostly made of calcium and phosphate ions, minerals which combine to form hydroxyapatite crystals. These crystals create a substance so strong that tooth enamel is the hardest part of the body—harder even than bones.

But it’s not indestructible! If you don’t care for your teeth with regular brushing and flossing, you’re leaving your enamel open to attack.

The bacteria in plaque convert the nutrients they get from the food we eat—especially sugars—into acids. These acids strip away the calcium and phosphate minerals which are the building blocks of our enamel, weakening the surface of the tooth.

Over time, these weak spots get bigger and deeper as bacteria and acids continue to attack the tooth surface. Eventually, cavities form, damaging the enamel surface, and, over time, even reaching into the dentin and the pulp inside the tooth. Once a cavity has formed, you need dental treatment to clean away the decay and repair the tooth structure.

The good news is that, when caught early, it’s possible to rebuild a weak spot in the tooth enamel before a cavity can fully develop.

  • Remineralization

While acids strip minerals from your enamel, your body is also replacing those lost minerals all throughout the day. How does this balancing act take place? With the help of saliva.

The very minerals which acids strip from your enamel are found in saliva. Saliva bathes your teeth with calcium and phosphate ions which, in turn, bond with your enamel to rebuild and restore enamel strength. When missing minerals are replaced like this, it’s called remineralization.

But this careful balance of demineralization/remineralization can become upset. If you neglect brushing and flossing, for example, plaque buildup will increase the amount of acids your teeth are exposed to. This means that your body won’t be able to replace the minerals as quickly as they are stripped away, and the result is a white patch on your tooth where the enamel has lost mineral strength.

Fluoride toothpastes and fluoride treatments are some of the most efficient ways to remineralize your teeth. Fluoride is attracted to the minerals in your enamel. It bonds with the tooth surface, and, once there, it helps attract the calcium and phosphate ions in your saliva to restore lost minerals and repair weak spots.

But fluoride does more than just restore your tooth’s strength—it improves it! Fluoride ions join with the calcium and phosphate minerals in your teeth to form fluorapatite crystals, which are even larger and stronger than hydroxyapatite crystals.

Early treatment is your best opportunity to reverse the early stages of tooth decay.
